Visible crew/equipment: When Jeff walks into the restaurant kitchen and finds Daphne waiting for him, in two wideshots facing Jeff the actor's green tape mark can be seen on the floor, in front of his foot. (00:40:05)

Continuity mistake: When Piper writes the name "Nell" backwards on the castle window she uses two fingers so each letter is doubled lined, but when the name is viewed from the outside all the letters are single lined. Additionally, after she writes the backwards "N" on the bottom right window, in the next closeup of the swords' pointy ends that window is blank. (00:30:20)

Revealing mistake: When Martin uses telepathy to tell George's girlfriend how to open the window, the two wires attached to the bows on the dog's floppy ears are visible. (00:05:05)

Continuity mistake: When Miss Topsey and Miss Turvey first confront Phil, Topsey holds up the I.O.U. with her left hand, but in the next shot it's being held up with her right hand. (00:08:00)

Deliberate mistake: After the intro, the first shot of Isabel as she starts shouting about not fighting is a flipped shot. Note everything is backwards compared to the following shots. (00:01:15)

Continuity mistake: While Jaime and the High Sparrow are standing beside Myrcella's body in the Sept, in the overhead shot of Myrcella her right hand is placed over her left hand, but in other shots it's the opposite way. (00:18:30)

Deliberate mistake: After Ramsey shows Sansa her flayed northern friend, when it cuts to Davos riding the horse through the encampment one of the shots is flipped. Note his sword and his cloak's crossed straps have switched to opposite sides.

Visible crew/equipment: While Phoebe begins her "fifteen minutes of channel surfing" she has the burning premonition and cries out, then in the next shot as Prue and Piper rush in, we can see the reflection of the boom mic/pole in the wall mirror. (00:02:10)

Visible crew/equipment: While the Book of Shadows flips its pages the sisters enter the attic, and after Piper asks how the book does that, in the next shot the feet of a crewmember can be seen moving quickly out of shot, at the bottom left side of the screen. (00:03:15)

Continuity mistake: While Phoebe explains the slam man training system Piper puts on Phoebe's boxing gloves, and when Prue says that demons don't play fair Piper playfully raises her left arm to Prue's forehead, but in the next shot facing Prue it's Piper's right arm that's raised. (00:04:00)

Continuity mistake: While Andy and Darryl are at the station talking about why Andy's been "walking around...like a zombie," Andy places a pencil behind his ear, but when it cuts to Inspector Blakely walking in behind Andy the pencil has vanished between shots, but it reappears again in the next shot. Then when Andy borrows the photograph of the knife, once again the pencil vanishes between the closeup and wideshot. (00:09:10)
