Character mistake: When Leon goes to see Susan Hoff with his photography portfolio she tells him he needs improvement. After she leaves, Leon tells Jurgis that he thinks she hated his work and Jurgis tells Leon "when she hates somebody's work she gives them about a minute, you got three." In reality, she actually did give him one minute almost to the second. (00:06:58)

Revealing mistake: After Mahogany is thrown off the train, as the train brakes and begins to stop, there is a shot of one of Mahogany's hung victims and you can see the whole body vibrate like rubber, which it was made out of. (01:21:47)

Continuity mistake: At one point, when Leon is fighting with Mahogany on the train, there is a shot of blood dripping off a knife onto Leon's face. This blood later vanishes. (01:20:02 - 01:21:19)

Visible crew/equipment: When Maya is searching Mahogany's apartment there is a shot of her looking in a bathroom cabinet mirror and you can see the arm of a crew member wearing a short sleeved white T-shirt reflected in it. (01:02:22)

Continuity mistake: Throughout the film we see Mahogany attack people with his hammer and blood will spray everywhere however Mahogany stays mostly or even totally clean. A good example of this is when Leon follows him onto the train and there is a shot of Mahogany, seen through a train door, repeatedly striking a victim with his hammer and blood is spraying all over him. He's totally clean a couple of shots later however. (00:51:12)

Visible crew/equipment: In the opening murder scene watch when the man is stood outside the connecting train car doors, there is a shot from behind, moving in on him, and if you look out of the left window you can see a set of yellow ladders. This is set equipment and the train obviously can't be moving. (00:02:20)

Continuity mistake: In the opening murder scene a large stripe of blood suddenly appears across the connecting train car doors after the man slips in the pool of blood. (00:01:18 - 00:01:42)

Visible crew/equipment: As Alex, Salvatore and Hesh walk along the bridge, if you watch the right side of the screen you can see a crew member wearing a purple T-shirt and holding a piece of equipment high in the air. (00:39:39)

Revealing mistake: When Tony buys the orange juice off the street vendor (just before he is attacked by the hitmen) you can see the magazines on the vendor's stand being blown around by the "heavy winds" that are present throughout the episode. However, if you look behind Tony, you can see the scenery is almost completely still. The "wind" is coming from just off camera. (00:27:17)

Revealing mistake: Throughout the film, Agent Rogersz is supposed to have a metal hand however there are many times when you can tell it is just a steel-coloured glove as her fingers move like real human fingers when they should be stiff. The films producers have stated that they could not find a convincing prop in time for shooting.

Revealing mistake: At the end of the film there are a number of shots of the luminous green Chevy Malibu were you can see its license plate is backwards. The shots are flipped.

Revealing mistake: When Tony collapses in his back garden, and the barbecue grill explodes, some coals (supposedly thrown into the air by the explosion) land next to Tony's body. Watch this moment closely however and you can see that the coals come into shot from left of the screen and not above from the air as they should. Obviously thrown in by a nearby crew member, just off-screen. (00:19:48)

Revealing mistake: When Artie visits Livia at the hospital there is a close-up of the newspaper that Livia is reading and it is obviously fake. Some of the articles look like they have been pasted into it and there are white strips of paper covering up headlines or replacing them with new ones. (00:16:50)
