Visible crew/equipment: Early in the the film, after the attack in a shot with police tape in view, you can see a camera on a dolly along with the focus puller in the left side of the shot (wide screen version). (00:11:55)

Continuity mistake: After Chris is dead, and his wife is writing in her diary, and she writes "I still exist" as she's writing it, exist is written "EXST", then when she looks to read what she wrote it reads "EXT". The writing's different too. (00:22:10)

Revealing mistake: In the scene where Jack Ryan uses the radio after the helicopter crash, the film is reversed - the word frequency appears as a mirror image.

Visible crew/equipment: When the Red October sub has surfaced to let the crew get off because of the supposed nuclear leak, there is a brief shot where you can clearly see the camera and its filming crew in their tennis shoes on the deck. (01:45:52)

Continuity mistake: When Roy breaks his bat in the playoff game, the bat is a long way from the batter's box; the "Wonderboy" fragment is lying further away from the foul line than the rest of the bat, and the fragment "point" faces away from the foul line, the top of the bat on both parts face the line. However, in the close-up, the fragment is facing the other way, with the point facing the foul line. Note, too, that when Hobbs hits the ball, the bat is not split. (02:09:10)

Continuity mistake: At the beginning of the film, the young Roy breaks a board of the hen-house wall with a pitch. When the shot cuts back to the hen-house wall a moment later, the hole is a different shape and in a different place.
