Big Deal in Paradise - S1-E21

Trivia: In an homage to the king of Hawaii-based cop shows, Hawaii Five-O, Don Ho tells MacBride and Ryan at the end that if they ever break up his night club again, he's going to call McGarrett. Ryan gives MacBride a baffled look and just before the freeze-frame, queries, "Who's McGarrett?"

Revealing mistake: During the flashback sequence showing Rocky's past crimes, two explosive devices are thrown at a shop window. Both bounce harmlessly off and land on the sidewalk, but the shop obligingly explodes anyway.

Continuity mistake: When George is chopping wood, his coat is either buttoned or unbuttoned, depending on the shot.

Factual error: George tap dances down the stairs after visiting the President in the Oval Office. But the Oval Office is on the ground floor of the White House.

Mr. Monk and His Biggest Fan - S6-E1

Plot hole: It's out of character for Monk, a chronic stickler for details, not to correct an error, especially one relating to his own life. Marcy's diorama for "Mr. Monk & the Three Pies" includes Natalie, who wasn't around yet when "Three Pies" occurred. The doll should be of Sharona, but since that would ruin the ensuing joke, Monk is allowed to overlook it. He's aware of Marcy's unreasoning obsessions, but Monk's OCD would simply not permit him to keep quiet about a mistake that glaring.

Continuity mistake: The shots of the tape recorder giving Phelps his assignment don't match. Each time the camera angle changes, the tape reels are spinning at different speeds and in opposite directions.

Dark Shadows (1966)

Continuity mistake: 1967: "Dark Shadows"' taping schedule seldom permitted retakes. Consequently, hundreds of goofs ended up on the air. Perhaps the funniest of them all occurred in a scene inside the Collins mausoleum, where the late Louis Edmonds was supposed to say, "Some of my ancestors are buried here." What came out was "Some of my incestors are buried here." To his credit, Edmonds not only kept his cool, but laughed and corrected the mistake in character.

Continuity mistake: During Phelps' mission briefing, the amount of tape on the left-hand reel, which should be diminishing as the assignment is read to him, increases instead. (00:01:45)

Continuity mistake: When the Major checks the prison log book, the box next to 27 where the time entry should be is blank. He closes the book and holds it under his arm while conversing with a subordinate, and when he opens it again to show it to the other man, the time box has, impossibly, been filled in. (00:43:30)

Death by Resurrection - S1-E12

Continuity mistake: McBride and Ryan are driving through town in heavy traffic. When the shot cuts (and their conversation continues uninterrupted), not only does the rear view mirror disappear, but an obvious rear screen projection changes their locale to a country road with trees and no traffic at all. The Mile High Job - S1-E8

Trivia: Someone on Leverage's writing staff is a Dr. Who fan. When the team is on the escalator at the airport, Hardison asks Nate over the ear piece, "What IDs have you got on you?" Nate replies, "Let's see, we've got Peter Davison, Sylvester McCoy, and I have a Tom Baker." Sophie chirps, "Oh! I have a Baker as well!" Nate and Sophie then board the plane as Mr. and Mrs. Tom Baker. This was the first of what would become a running gag: the Tom Baker alias and several other Who references recur throughout the series. (00:12:00)

Trivia: A number of in-joke Dr. Who references crop up throughout the series pilot. A group of doctors and scientists are introduced as Hartnell, Troughton, and Baker; there's a business card for I. M. Foreman in Totters Lane; and the cover of a science magazine sports headlines such as "Time and Relative Dimension in Space" and "Playing the Game of Rassilon."

The System - S3-E15

Continuity mistake: Phelps takes the tape from the recorder's right-hand spindle. When he opens the acid vat, we can see the tape he just removed back on the recorder - but on the left spindle. The tape he then drops into the acid doesn't remotely resemble the one he pulled, and when he drops it, the recorder's left spindle, where the full tape was in the last shot and where the empty reel should be, now has nothing on it at all.
