Revealing mistake: When the Captain is tripped over by Ricky, his face lands on a plank of wood with nails sticking out of it. In the shot of his face getting impaled with the nails, we can see that the Captain has been replaced with a dummy. (00:08:50)
Other mistake: Just after the warden tells the guard to limit the amount of food the inmates get in their meals, one of the inmates shown polishing the meat grinder says "You eat like a lousy king. Even your dog gets a big steak." When he says "your dog", the subtitles read "yourdog" instead of "your dog." It has been incorrectly spelt as one word, with no space shown in between the two words. Other mistake: Just after the warden tells Ricky they are trained in the same kung fu style, Ricky thinks to himself for a moment. The subtitles read 'If your opponent is strong, use your wits to beat him', but before the start of the line, before the word "If", a randomly placed comma can be seen at the start of the line. There is no reason for this comma to be here since there are no words shown before it. Clearly this was an unintentional error made when the subtitles were being typed out. Other mistake: When Hai is speaking with Kuang just after stopping him from killing the bald inmate, Hai asks Kuang if he kills him, what do they say to the assistant warden. There is an error involving the English subtitles regarding Kuang's next line. Kuang's line, in the subtitles, is listed across two rows, but the rows are displayed in the wrong order. The first row reads "him, we won't take it anymore." And the second row reads "Hai, lots of men have died because of", which makes no sense when it's all put together. The full line should have been displayed as "Hai, lots of men have died because of him, we won't take it anymore." But it has incorrectly been displayed as "him, we won't take it anymore. Hai, lots of men have died because of." (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). Visible crew/equipment: During the headstone smashing scene, there is a moment right after breaking one with his back, where Ricky does a backflip and destroys a headstone being thrown in the air, and the hand of a crew member can be seen momentarily to the right of shot when it is thrown at him. Revealing mistake: When the prison wall begins to break apart after Ricky punches it, in the wide shot where the debris from the punch is shown falling to the ground, multiple wires can be seen pulling the chunks of debris from the wall. (01:30:04)
Revealing mistake: When Ricky throws the warden into the meat grinder, in the shot where the warden is being thrown across the kitchen, a wire can be seen attached to him, hoisting him across the room. (01:28:37)
Continuity mistake: When Ricky is trying to force the warden's body into the meat grinder, as Ricky begins pushing his head down, the warden is positioned directly in front of Ricky. When the camera changes angles, the warden has suddenly rotated by 90°. The camera changes angles again and the warden is suddenly facing Ricky directly, then in the next shot, the warden has suddenly rotated 90° again. (01:28:53)
Continuity mistake: When Ricky begins to charge towards the wall to punch it down, his right fist is shown to have blood on it. When the camera cuts to angle behind Ricky, the blood on his right fist has suddenly disappeared. (01:29:55)
Revealing mistake: Just after the warden throws Ricky to the ground, when he finishes saying "Your strength doesn't even come close to mine", he starts doing an evil laugh and here, we can see the actor's real tongue and teeth behind the fake prosthetic suit that he is wearing. (01:28:02)
Revealing mistake: When Ricky jumps over the table after the warden pushes it at him, there is a shot of Ricky kicking the warden with his foot whilst in mid-air. In this shot, we can see that Ricky's trousers are being pulled upwards, revealing that there is a wire attached to his trousers that is pulling him into the air. (01:26:30)
Continuity mistake: When the warden is kicked across the kitchen by Ricky, in the shot where he lands on a sink, there are two circular smudge marks on the wall beside him. It then cuts to a closer shot of the warden and the two smudge marks have gone. (01:26:31)
Continuity mistake: When Ricky is sitting on the ground after being thrown to the ground by the warden, whilst the warden is telling him they've got the same kung fu style, Ricky has both his arms placed firmly on the ground. The camera changes angles and suddenly, his left arm is raised off the ground. (01:28:05)
Continuity mistake: When the warden is kicked across the kitchen by Ricky, he lands on a sink. On the floor beneath the kitchen sink, a couple of tiles that were stuck to the wall fall off it. We are then shown a closer shot of the same area, where the tiles that just fell off the wall are now back on the wall, and two entirely different tiles have now fallen off. (01:26:31)
Continuity mistake: When the warden is preparing to fight Ricky, he takes his belt off and drops it on the tray of meat on the table in front of him. The warden then pushes the table towards Ricky and the belt is now positioned to be beside the meat tray. Ricky then kicks the warden across the room and the belt is now suddenly nearer to the other end of the table. (01:26:28)
Continuity mistake: Just after the warden has one of the guard take his son away, he and Ricky prepare to fight. At this point, Ricky has no blood shown around his mouth. But later in the scene, when the warden becomes muscular, there is suddenly some blood around his mouth that wasn't there earlier. (01:26:20 - 01:27:14)
Continuity mistake: The warden attempts to trap Ricky by pushing a table at him. On the table there is a tray filled with meat from when the warden forced an inmate's arm into a meat grinder earlier in the scene. In the shot where he first pushes the table, the meat in the tray is towards the right hand corner of the tray. Ricky then jumps over the table and the meat is now towards the left hand side. Revealing mistake: When White God fires a needle through the wall of the kitchen, Ricky grabs onto it and is dragged towards the kitchen wall. Just before he crashes into the kitchen wall, a large outline can be seen in the wall, revealing where the effects team made a hole in the wall and plastered over it before filming. (01:23:38)
Other mistake: Just after the assistant warden has been carried away by the other inmate, it cuts to the warden in the kitchen with his son and a guard. When the guard tells him the menu for tomorrow is ready, the subtitles "Sir, the prisoner's menu for tomorrow is ready." The apostrophe in "prisoner's" is shown in the wrong place. Saying "prisoner's" implies that it's just for one prisoner, and since the menu is made for all of the prisoners, the subtitles should have read "the prisoners' menu" and not "the prisoner's menu." The guard even says "They'll have some beans, vegetables and meat for lunch" afterwards, so multiple people were being referred to.
