Although you find out that the detective's girlfriend, Beth, the psychiatrist played by Jeanne Tripplehorn, had an unhealthy obsession with Sharon Stone, it turns out that Sharon Stone is the murderer... the last shot is the ice pick under her bed. Also that was Catherine Trammell and not Beth murdering the guy in the opening scene. Catherine framed Beth for it all.

I wouldn't say that she had an "urge" to kill. Rather, she was willing to "dispose" of people by killing them. She would kill people who were in her way or were no longer interesting/useful.
