In the Mouth of Madness ending / spoiler
John Trent (Sam Neill) escapes Hobb's End and that's the end of the story he's telling at the mental asylum. The more people read Sutter Cane's (Jurgen Prochnow) books, the more twisted the world becomes. Moments later, everyone in the asylum except John is slaughtered by Sutter Cane's creatures. John leaves the asylum, wanders around town and there's no one around because everyone's gone mad and killed one another from reading Cane's books. John goes into a movie theater that's showing the movie In the Mouth of Madness (based on Cane's book). The movie that John watches is this movie and he starts laughing hysterically...
